The AFL allowing Collingwood to cheat and get Luke Ball.
The umpires deciding there was two sets of rules for Gary Ablett as a forward in the 2009 GF.

Swapping Ian Stewart for Billy Barrott. And I reckon the Ball/Judd one might've cost us about 4 premierships.

Assuming Luke Ball is worth one point over the course of 100 minutes then RL encouraging him to walk to Cwood cost us a flag.

Leaving Ball on the bench for the last quarter in 09 grand final

Action:
Collingwood not turning up for their Prelim in 2009. Cats got an easy ride while we fought the Dogs to the wire.
3/4 time huddle on Grand Final day and our guys were spent. Geelong were always going to run over the top.

Lovett is obvious one to me. Wasted a spot for a good player.
Controversial one is I believe we would have won if we took cousins when we could. Put in cousins, take out one of those 'bottom3' and the side improves.
And yes I see the irony between those 2

Their inaccurate kicking for goal in the 2009 GF.

God deciding to make it rain in '09. A half decent day and Roo and Kosi were primed to dominate that day.

Re: Decisions and actions that cost the Saints a Flag
One thing that comes to mind is how bad our drafting was during these years. Most premiership sides have at least one young gun in their side somewhere.
Where was our Hannebery, Steele Sidebottom or Brad Hill or even our Lachie Neale or Christensen?

Hanging on too long to Grant Thomas
I reckon Lyon would've got us a flag with a side containing some of the mature names, Hamill, Grhrig, Harvs, etc with some of the top line kids we also had coming through...
Alas..

Two dumbass bits of bad luck, the Scarlett toe poke and the ball bouncing at 90 degrees.
Problem is, we can't change it, so no use dwelling on it.
Oh and Alves leaving Shanahan on Jarman.
